Terms and conditions: Completing this form does not guarantee that I will be able to provide you with the memorial portrait service, and is subject to availability. I will try my absolute best to accommodate memorial requests, due to the importance of the service provided. Upon checking my availability, I will get in touch with you ASAP to book in. As soon as booking is made, I will raise an invoice to be sent to you. Due to the short time frame, I require a 50% deposit before work commences, with the remainder due upon completion.
